The Battle Over Bone Marrow Transplant And How To Win It
There are three kinds of bone marrow transplants:
A stem cell transplant is done after chemotherapy and radiation is complete. The stem cells are delivered into your bloodstream through a tube called a central venous catheter. The process is similar to getting a blood transfusion. The stem cells travel through the blood into the bone marrow. Usually, no surgery is required.
In many cases donor stem cells can be collected directly from the person's blood. Surgery is not needed. The donor will first receive injections for a few days.
Minor surgery may be needed to collect bone marrow and stem cells from a donor. This is called a bone marrow harvest. The surgery is done under general anesthesia, which means the donor will be asleep and pain-free during the procedure. The bone marrow is removed from the hip bones.
After entering the bloodstream, the stem cells travel to the bone marrow, where they begin to produce new white blood cells, red blood cells, and platelets in a process known as 'engraftment.' Engraftment usually occurs within about 2 to 4 weeks after transplantation. Doctors monitor it by checking blood counts on a frequent basis. Complete recovery of immune function takes much longer, however-up to several months for autologous transplant recipients and 1 to 2 years for patients receiving allogeneic or syngeneic transplants.
Doctors evaluate the results of various blood tests to confirm that new blood cells are being produced and that the cancer has not returned. Bone marrow aspiration (the removal of a small sample of bone marrow through a needle for examination under a microscope) can also help doctors determine how well the new marrow is working.
